What type of information we have

Information regarding applicants to the Musicians’ Hearing Health Scheme & Music Venue Trust.
How we get the information and why we have it
Personal information that we process is mostly provided directly by you, however there are other purposes for the retention of data which could include the following:
For the tailoring of audiological advice and delivery of services and products.
Information to assist in treatment received from 3rd parties such as medical consultants and doctors.
Information received from relatives or friends with prior approval.
We also receive personal information indirectly, from the following sources in the following scenarios:
Information regarding the Musicians’ Hearing Health Scheme (MHHS) supplied by Musicians’ Hearing Services.

What we do with the information we have
We use the information that you have given us in order to:
Contact patients to arrange appointments and onward delivery of products ordered.
We may share this information with Musicians’ Hearing Services (only if you are being seen on the MHHS scheme), your General Practitioner (once verbal consent has been obtained during your appointment) & any other relevant health or social care providers (again only with your verbal consent).
Manufacturers and suppliers of any ordered goods that might be delivered to you.
How we store your information
Your information is securely stored electronically on our clinical patient management database, our mail server / drive. These are both highly secure, end-to-end encrypted services which are fully GDPR compliant.
We keep your personal information indefinitely, unless you request we dispose of it. Where data is to be destroyed, paper records are shredded and disposed of appropriately. Electronic records are deleted or archived, which restricts all access.
